Looked at the quest log and her crew mission is botched at the "Find Parvati Fancy Clothes" part.

I'm standing on Scylla and she is in my party, not dead. I have the same problem. Nyoka is still very much alive, but the quest is botched. I've had the same problem with Nayoka and Parvati . Such a shame, really breaks the immersion for me.
